Diagnostic Peritoneal Lavage

The concept of lavage as an effective means for detecting intraperioneal injury secondary to trauma has been adequately demonstrated. The value of peritoneal lavage is especially reflected in the statics reporting approximately 20% incidence of unnessary celiotomies when physical examination findings alone are used, where as the number of false-positive and false negative may be lowered significantly. Not only dose peritoneal lavage decrease the number of unnecessary celiotomies, but it also provides for a more rapid diagnosis that can prevent greater blood loss. Thereby reducing the number of subsequent transfusions, minimizing chances of hepatitis, and lowering the incidence of shock with its coincident complications. We present the clinical and the literatural review of 63 cases of the peritoneal lavage from Jan-1-1978 to Aug-31-1979 at C.N.U. Hospital, the Department of the Surgery. The result as following. 1) There is 54 males and 9 females in sex. The age of incidence are 16 patients below 15 years, 47 patients above 15 years. 2) There is positive 51 cases, negative 11 cases, false-positive, but false-negative none. 3) In 51 positive patients, there are 34 cases above 20 ml blood on aspiration from the peritoneal cavity, 12 cases above 100,00/mm RBC, 17 cases above 500/mm WBC and 2 cases above 175/dl amylase. 4) The accuracy is 98.4%. 5) Injurid organs, there are spleen 13 cases, small intestine 12 cases, liver 11 cases, retroperitoneal hematoma 10 cases and multiple injuries are 11 cases. 6) We can decrease the morbidity and mortality rate by early diagnosis with perioneal lavage.
